“Infertility,” a harmless, insubstantial show about the struggles of five people hoping to become parents, bills itself as “the musical that’s hard to conceive.” Not really. After “Slut,” “Dr. Sex” and countless other sex-themed Off Broadway musicals, it seems only natural that a show about getting pregnant would emerge.

Written and composed by Chris Neuner, “Infertility” had its premiere – where else? – at the 2004 New York International Fringe Festival. It is now at Dillon’s restaurant, where it is a light side dish to go with the calamari and pasta on the menu.

A missing link from obesity to infertility: Researchers unravel clues to infertility among obese women

ScienceDaily (Sep. 8, 2010) — Obese women have a well-known risk for infertility, but a new Johns Hopkins Children’s Center study has unraveled what investigators there believe is the mechanism that accounts for the risk.

The research, conducted in mice and published online on Sept. 8 in the journal Cell Metabolism, shows that the pituitary gland actively responds to chronically high insulin levels, triggering a cascade of hormonal changes that disrupt ovarian function and impair fertility.

The findings challenge the widely held belief that infertility is a result of insulin resistance — a body’s insensitivity to chronically elevated insulin levels and a hallmark of obesity — and suggest a new culprit: heightened sensitivity to insulin’s effects on the pituitary gland.

Laughter may not be the best medicine, but it might help women who are trying to become pregnant through in-vitro fertilization (IVF), a small study suggests.

In a study of 219 women undergoing IVF, Israeli researchers found the odds of success were greater among women who were entertained by a professional “medical clown” right after they had the embryos implanted in the womb.

Newbie seeking info on immune testing

Dr. Coulam,

I am so glad to find a place to ask questions about getting started with immune testing. Brief history- I was dx with severe endo. in 2006 which was surgically treated. Both tubes were found open though. No symptoms since then. I am now TTC for almost 5 years mostly doing IUIs and on our own till 2007. Failed an IVF cycle in 07 and then another and again FET in 08. Each time perfect looking grade A blasts were transferred. No pregnancy. I have never had a + pg test. I strongly suspected immune issues causing repeat implantation failures. Tested for APA’a, Lupus, ATAs, ACA by my local RE . All normal. Due to start with another IVF soon out of state and have a strong gut feeling to get tested for immune issues with my next cycle. I know after reading info on the threads here that I can consult with you over phone and have the tests prescribed. Before going ahead, a couple of questions if you could please answer:

Laparoscopy Diagnosis and Treatment of Infertility

Laparoscopy A laparoscopy forms the cornerstone for the surgical evaluation of the pelvis of an infertile woman after a basic infertility investigation is completed. In some cases where endometriosis, pelvic inflammatory disease, or pelvic pain needs evaluation, it may be indicated earlier in the evaluation. The laparoscopic procedure is sometimes combined with a hysteroscopy when indicated. LaparoscopyLaparoscopy is performed under general anesthesia in a surgical center. A small needle is inserted into the abdominal cavity and C02 gas is introduced. The laparoscope (a telescope with a strong light and camera) is introduced through the navel. A variety of instruments can be inserted into the pelvis, through other small incisions, to perform diagnostic and corrective surgery.

This procedure allows Dr. Kustin to visualize the internal organs where reproduction occurs. In every case, he undertakes a meticulous surgical evaluation of the pelvis documenting his findings by video (it is routine for Dr. Kustin to review this video with his postoperative patients so that they may gain a first hand understanding of their surgical problems).

As part of the surgical evaluation the following are examined:

  • Evaluation of the tubes and ovaries.
  • Exclusion of endometriosis, both overt and subtle
  • Identification of adhesions
  • Inspection of the uterus and the exclusion of fibroids
  • Inspection of the appendix, intestine, gall bladder, liver, and upper abdominal cavity

As complicated as infertility may get, we may find that it can present no real symptoms that will help clear the condition of a couple who are incapable of conception. When this is the case, doctors may diagnose “unexplained infertility”.
